GCIRP offers supportive services for people who are incarcerated in preparation for, during and after the process of re-entry.

Our team of case managers and other service providers help clients successfully transition back into the community, reunify with their children and families and prevent a return to incarceration.

GCIRP’s case managers are here to…  

...help clients and their families determine specific short- and long-term goals, as well as plan the steps to meet those goals and link clients and their families to community service providers for additional support with the following:
  • housing
  • drug and alcohol treatment
  • mental health services
  • medical care
  • employment
  • counseling

GCIRP's approach includes…

Community-based services

GCIRP case managers begin working with a client prior to release, creating a release plan to address their specific needs and concerns. The case manager also provides continued support as the client transitions back into the community.

Client advocacy

GCIRP case managers stand alongside clients, supporting and encouraging them to express their needs, make their own decisions, contribute to their community and work through common re-entry barriers.

Linking to community partners

GCIRP case managers have working relationships with committed community partners. This means the Case Managers can link clients with the programs and people that can help them find employment, housing, education, counseling and more.

Family involvement

GCIRP case managers not only work with the client, but also their biological family members and/or other support partners. The goal is to strengthen the bond between the client and their supportive network, which can help everyone involved with the re-entry process.

Eligiblity

The GCIRP Initiative is open to adults who…
• have 3 to 6 months of incarceration left before release
and
• will be living in Cuyahoga County after release

GCIRP's case management partners are the Center for Families and Children and Community Re-entry. The project is generously funded by the Cleveland Foundation, Cuyahoga County, Health and Human Services - Office of Reentry, the George Gund Foundation and United Way of Greater Cleveland.
